Volta à Noruega em farmácia // Tour of Norway in Pharmacy

A partir do corrente mês de Julho "mudei" de emprego. Continuo ligado contratualmente à empresa Apotek1 - também não podia ser de outra forma porque temos de cumprir obrigatoriamente 1 ano de contracto - mas como estava a trabalhar na minha farmácia apenas para suprir a falta de uma farmacêutica que estava de baixa de maternidade, foi necessário arranjar uma outra solução.

Farmácias na Noruega #2 // Pharmacies in Norway #2

Vamos então falar um pouco sobre o nosso trabalho como farmacêuticos na Noruega, sempre na óptica de quem veio de Portugal e lá trabalhou.

Como comecei por abordar neste post ainda no jardim a beira mar plantado que nos viu crescer, esperávamos grandes diferenças em muitos aspectos do trabalho como farmacêutico.

É impossível passar por cima do respeito que há aqui pelas regras. Por exemplo, se um determinado medicamento é sujeito a receita são raras as excepções que se abrem para o vender sem existir uma receita. Nesta Sexta feira tive um caso em que um utente foi à farmácia tentar comprar uma embalagem de Ventilan Diskus (inalador usado para problemas respiratórios), para o qual até tínhamos registo de ter vendido a esse utente algures em Novembro.

Pedaços de um infância perdida // Pieces of a lost childhood

Parece que ainda andamos por cá. :)
Na sexta feira passada acabámos o nosso tempo para aprender o sistema e um pouco das regras de comparticipação e dispensa de medicamentos na Noruega.
Como despedida o pessoal da farmácia ofereceu-me umas prendas, o que foi bom mas surpreendente. Mais surpreso ainda fiquei quando vi o que me tinham oferecido:



Claro que tem uma história por trás. Quando eu era "pequenino" por volta dos meus 18/19 aninhos, pedi ao meu velho Pai um destes helicópteros numa loja grande de brinquedos que havia no Dolce Vita Tejo. Eram tempos duros, em que não havia muitos euros no meu bolso, e o vilão não me comprou o malfadado brinquedo. Desde essa altura sempre quis ter uma coisa destas mas nunca a encontrei pelo mesmo preço. Então já na Noruega durante o almoço vi isto no catálogo de uma das lojas do centro onde estava e comentei com os colegas que ia comprar um coisa daquelas. E no dia seguinte tinha isso à minha espera. Agora já está a Carol um bocado farta do zumbido do heli :P

A Carol está também a entrar em contacto com o seu lado infantil e arranjou isto:





Apresento o Inútil, um(a) hamster gordo pelo qual a Carol anda a melgar-me à tempos e que comprou pela net. Portanto neste momento somos três cá em casa. Facilmente sabem quem é que baptizou o (a) bicho não? :)

Estamos na nossa primeira semana nas farmácias onde efetivamente vamos ficar a trabalhar. Com uns agradáveis -4/-5ºC de manhã quando vamos para o trabalho.
Sobre a minha, posso dizer que é uma farmácia muito pequena, com apenas 8 empregados mas não estão todos a full time.  Acaba por ser também muito parado e muito "calmo" a nível de movimento, mas tem bastante trabalho de backoffice. (O que é uma seca).
Mas a integração não podia estar a correr melhor todos muito agradáveis e prestáveis de maneira a que a que as várias perguntas que fazemos ao longo do dia são respondidas sem problemas.
A Carol tem feito uns horários menos estáveis, enquanto que eu faço sempre das 9 às 17h. O que fez com que hoje estivesse sozinho até quase às nove da noite, e a malandra da Carol nem me deixou jantar feito. Como é que ela foi capaz?? Bem, pelo menos havia cereais x).

It seems that we are here :)
Last Friday we finished our time to learn the system and some of the rules of reimbursement and dispensing of medication in Norway.
As a farewell the pharmacy staff offered me a gift, which was good but surprising. More surprised still was when I saw what I had been offered as you can see in the image above!


Of course it has a story behind. When I was a "little" about my 18/19 :P, I asked my old Dad to buy me this kind of helicopter in a large toy store that was in the Dolce Vita Tejo (near Lisboa). Hard times, when there were not many euros in my pocket, and the villain didn't buy me the ill-fated toy. Since then, I always wanted something like this but I never found at the same price. So already in Norway during lunch, I saw it in the catalog of a downtown shopping where I was having my internship and commented to my colleagues that I would buy a thing like that. And the next day had it it waiting for me. Now Carol is kind of sick bit of the heli Buzz: P

Carol is also to come into contact with your child aside and got this:


I present you Inútil ("Useless") one fat hamster by which Carol was bugging my ears about and that she bought in the internet. So right now we are three here at home. Easily to know who has given  the name to the animal... right? :)

We are in our first week in the pharmacies where we actually work. With a pleasant -4 / -5 ºC morning when we go to work.
About mine, I can say that is a very small pharmacy, with only 8 employees but not all working the full time. Turns out to be also very still and very "calm" in terms of movement, but has enough work backoffice. (What is a drought).
But integration could not be going better. They are all very nice and helpful so the variety of questions we do throughout the day are answered without problems.
Carol has a less stable schedule, as I always do from 9 to 17h. What made that today I was alone until nearly nine o'clock at night, and the naughty Carol not even let me dinner done. How she was able ?? Well, at least I had cereal x).

Farmácia na Noruega #1 // Pharmacy in Norway #1

Amigos farmacêuticos portugueses se estiverem na farmácia e seja necessário Dermovate, liguem-me que eu tenho muitos com papelada Portuguesa cá.

Portanto, aqui é perfeitamente normal haver importação paralela e não só de Portugal, como também da Suécia por exemplo. Ficaram foi muito surpreendidos quando contei que por vezes temos falta de medicamentos devido a esta exportação paralela, e  nem sei se há falta de deste produto actualmente.

Acabámos hoje a nossa primeira semana de "estágio" mas a minha semana foi muito diferente da semana da Carolina. Enquanto eu passei a semana toda de volta do balcão sem aprender praticamente nenhum trabalho de backoffice, a Carol esteve este período a aprender essa faceta do trabalho de Farmacêutico na Noruega.
Há muitas, muitas diferenças no trabalho realizado por nós como farmacêuticos. Algumas coisas vamos contando mas, mais tarde e com mais experiência, faço um apanhado com uma comparação com o nosso trabalho como Farmacêuticos em Portugal.


 Continuando na mesma temática, sabem o que é que é realmente muito caro?
- Os medicamentos!  É só um exemplo mas hoje vendi com receita uma caixa de Xanor 2 mg de 100 comprimidos ( que é uma quantidade absolutamente normal aqui), por 353 NOK. O que equivale a mais coisa menos coisa 42 Euros por uma embalagem de Xanax Xr (Alprazolam) em Portugal, que ficaria a cerca de 10€ a embalagem de 60.
Claro que há vários tipos de receitas e de comparticipações e a maioria das pessoas nesta altura já atingiu o máximo que pode pagar por certos medicamentos e assim, pode levá-los de graça.

EXTERNAL USE means nothing in Norwegian, I can assure that :P
So, here it is perfectly normal to have parallel importation not only of Portugal but also from Sweden for example. The pharmacists here were even quite surprised when I told them that sometimes we have lack of drugs because of this parallel exports. I don't even know if there is a shortage of this product currently in Portugal.

Today we finished our first week of "internship" but my week was very different from the week of Carolina. While I spent the whole week back from the counter without learning almost nothing from the work we have to do in the "backoffice", it was what Carol has deed in this period and she learned this facet of the pharmacist job in Norway.
There are many, many differences in the work done by us as pharmacists. Some things we will tell now but later and with more experience, I will do a roundup with a comparison with our work as pharmacists in here and in Portugal.


  Following on the same theme, do you know what is really overpriced?

- Drugs (legal ones :P) ! It's just an example but I sold  today a recipe with a box of Xanor 2mg 100 tablets (which is a perfectly normal amount here) for 353 NOK. Which is around 42 Euros for a pack of Xanax Xr (Alprazolam) in Portugal, which would be about 10 € to 60 packing.
Of course there are several types of revenue and contributions and most people at this point has reached the maximum you can pay for certain drugs and thus can get them for free. But nevertheless... it is really expensive!

So I can say now that we finished our first full week in Skien!

Sistemas de Farmácias na Noruega e comparação com o Português (antes de chegar) // Pharmacies' System in Norway and comparison with the Portuguese one (before arriving Norway)

Vou aproveitar para falar um pouco sobre o sistema farmacêutico na Noruega. 

Aviso que esta é a nossa visão antes de chegarmos lá :)


Até 1 de março de 2001 a possibilidade de ser dono de uma farmácia na Noruega estava vedada a quem não fosse Farmacêutico. Após essa data houve a liberalização total do mercado das farmácias possibilitando o aparecimento de grandes cadeias. As únicas restrições neste momento são para os médicos e para as industrias farmacêuticas. Podemos fazer a analogia com o que se passou em Portugal mas em comparação foi muito mais agressivo. Com esta liberalização foi possível um aumento brutal do número de farmácias, cerca de 40% em 6 anos(!). Expansão essa que se continua a verificar, explicando assim o porque de estarem com uma falta enorme de recursos humanos na área. 
As grandes cadeias vieram para ficar e estabeleceram-se sem dar qualquer tipo de hipótese. Neste momento cerca de 90% do mercado está dividido apenas por três empresas:
  1.  Apotek 1 é a maior rede de farmácias com uma quota de cerca de 45 por cento do mercado. Os cerca de 3.000 funcionários estão distribuídos por mais de 300 farmácias em todos os municípios e dois armazéns grossistas. É uma filial do alemão Phoenix Group
  2.  Boots farmácia foi lançado na Noruega em 2008 e atualmente tem 150 farmácias por todo o país. Filial da marca Inglesa homónima.
  3. Vitusapotek é uma rede de farmácias em todo o país, com 200 farmácias e 1.500 funcionários que tem uma filial que controla 23 farmácias hospitalares. Filial da alemã Celesio.


O paralelismo com o nosso futuro está aqui. Tenho a convicção que dentro de algum tempo estaremos numa situação parecida em termos de mercado mas muito pior em termos de emprego farmacêutico. Em Portugal as leis são apenas meras linhas orientadoras, a regulação não existe e a promiscuidade da OF com a ANF faria corar de vergonha qualquer Casanova. 
Na Noruega não se podem comprar Ben-u-ron 1000g como se fossem pastilhas, sem prescrição. Pílulas contracetivas sem receita?  É melhor nem pensar nisso. Ben-u-ron 500g? Uma por pessoa e nada mais. Nem vale a pena falar de Ansiolíticos ou Antibióticos que muitos colegas nossos andam a dar às pessoas se "for cliente habitual". 
E tudo começa por aqui. Se há regulação não acontecem casos de estarem pessoas SEM FORMAÇÃO NENHUMA a dispensar receitas. Na Noruega TODAS as receitas precisam de ser validadas por farmacêuticos. Há verdadeira responsabilidade pelo acto farmacêutico. 
Se realmente houver a liberalização do mercado em Portugal, não vai demorar muito até termos Técnicos  auxiliares de Farmácia  ou pessoas sem formação a atender e aconselhar utentes. Mesmo que os ordenados para farmacêuticos venham por ai a baixo, ainda mais.

I'll just talk a little about the pharmaceutical system in Norway. 

I have to say that this is our view before we arrive Norway.

Until March 1, 2001 the possibility of owning a pharmacy in Norway was allowed to Pharmacists. Thereafter there was a full liberalization of pharmacies enabling the emergence of large chains. The only restrictions right now are for doctors and pharmaceutical industries. We can make the analogy with what happened in Portugal but in comparison, I think that was much more aggressive. With this liberalization was possible a brutal increase in the number of pharmacies, about 40% in 6 years (!). Expansion that continues, explaining why they have a huge lack of human resources in the area.

The big chains are there to stay and settled without giving any kind of hypothesis. At this time about 90% of the market is divided only by three companies: 

  •   Apotek1 is the largest pharmacy chain with a share of about 45 percent of the market. The approximately 3,000 employees are spread over more than 300 pharmacies in all municipalities and two wholesale warehouses. It is a subsidiary of the German Phoenix Group;
  • Boots pharmacy was launched in Norway in 2008 and currently has 150 pharmacies across the country. English branch of the homonymous brand;
  • Vitusapotek is a network of pharmacies across the country, with 200 pharmacies and 1,500 employees who have a branch that controls 23 hospital pharmacies. Norwegian subsidiary of german group Celesio. 



The parallels with our future is here. I am convinced that over time we will be in a similar situation in terms of market but much worse in terms of pharmacist employments. In Portugal the laws are just mere guidelines, the regulation does not exist and the promiscuity of with the ANF (nacional pharmacists association) would blush with shame any Casanova. 

In Norway we can not buy Ben-u-ron 1000g as if they were pills without prescription. Contraceptive pills without prescription? It's better not to think about it. Ben-u-ron 500g? One per person and nothing more. Not even worth talking about anxiolytics or antibiotics that many of our colleagues go to give the people "is a regular customer." 
And it all starts here. If regulation does not happen there are cases of people WITHOUT ANY TRAINING to dispense recipes. In Norway ALL the recipes need to be validated by pharmacists. There is true accountability by the pharmaceutical act. 
If there really is a market liberalization in Portugal, will not be long until we auxiliary of Pharmacy Technicians or untrained to meet and advise users. Even though the salaries for pharmacists come around for a down further.

Going north II - Processo de Recrutamento // Recruitment Process

No seguimento do meu outro post,

Quando fui à segunda entrevista, falei com a directora de recursos humanos da empresa que ficou agradada com a possibilidade de em vez de ser apenas uma pessoa, passarmos a ser um casal a concorrer em conjunto. Para eles isto tem vantagens óbvias em comparação com alguém sozinho em termos de adaptação e possível permanência por tempo indeterminado. 

O processo de recrutamento utilizado por eles consiste no seguinte:

1.    Envio de CV em inglês com carta de motivação:
·      No meu caso, porque como tinha dito no post anterior fiz a candidatura por impulso, nem foi muito esforçada, uma adaptação rude da carta para o programa do EUA e o CV em inglês feito para essa ocasião que nem estava actualizado nem tinha fotografia.
2.    Primeira entrevista por Skype:
·      O normal: Quem somos, o que queremos, onde nos vemos dentro 3-5 anos. Explica as condições exigidas e o que é dado por eles.
3.    Teste Psicotécnicos 
·      Através de um site da internet. Algo extenso mas igual a muitos outros.
4.    Segunda entrevista
·      Análise um pouco exaustiva dos testes feitos. Porque que respondemos assim nesta questão? Porque não de outra maneira? O resultado do teste diz que é assim, o que tem a dizer sobre isto?
5.    Contacto com as referências
·      Algo que levaram muito a sério foi a questão das referências. Soube que contactaram o meu local de estágio várias vezes até conseguirem falar com o meu orientador. Ficámos com a impressão que sem referências sólidas não estaríamos agora nesta situação. 

Demorou cerca de três semanas até sabermos o resultado final. Por essa altura ambos tínhamos trabalho e como tínhamos duvidas de como nos iríamos adaptar continuámos nas respectivas farmácias até nos podermos assegurar que nos iríamos dar bem com a língua.

Following my other post, 

When I had the second interview, I spoke with the director of the human resources of the company and she was pleased with the possibility of being a couple competing together instead of being just one person. For them, having a couple has obvious advantages compared with someone alone in terms of adaptation and time of staying in Norway. 

Their recruitment consisted of : 

1 Sending CV in English with motivation letter: 
  • In my case, because like I said in the previous post, I made the application on impulse,without hardworking, and a rough adaptation of the letter to the program of the USA; CV in English made ​​for this occasion that was neither really updated nor had photo. 

2 First interview by Skype: 
  • Normal: Who we are, what we want, where we see ourselves within 3-5 years. They explained the conditions required and what is given by them. 

3 "Psychometric" Test:
  • Through an internet site. Something extensive but like many others. 

4 Second Interview 
  • Exhaustive analysis of the tests. why did we answer this in that question? Why not otherwise? The test result says "this" so, what do you have to say about this? 

5 Contact references 
  • Something that they took very seriously was existence of references. I Know they contacted the pharmacy where i did my internship several times until they could speak to my supervisor. We got the impression that without solid references it would be to be in this situation. 


It took about three weeks before we know the final outcome. By that time we were both working and we were trying to figure out how to handle with that!

So we started our connection with Apotek1 :)
